What kind of ID do I need to bring to get a replacement g1 in Ontario?

I lost my g1 drivers licence and I need to get a new one, I've called the office near me and I just keep getting a stupid recording and can't actually speak to anyone to ask. Can I get the replacement with just a health card and a birth certificate? Also how much does it cost? I've heard that it's $10.
